Which autoimmune disease or other process causes big toes to suddenly deviate into other toes, creating painful bunions, and the pisiform bones in wrists to enlarge at the same time?

??: None that i know of but what you describe could be related to various types of arthritis including rheumatoid although it could be a result of good old osteoarthritis too/ might be best to be seen and evaluated by a doc for this.

Destructive joints: There are inflammatory arthritites that can cause deviation and destruction of small joints: rhuematoid arthritis, psoriatic arthritis, etc. See your doctor for a referral to a rheumatologist if you are concerned. Hope that helps!
