Trauma or Infection : Dark and swollen nipple could indicate recent trauma with underlying hemorrhage and edema which can occur after intense love-making. It could also mean infection of the breast ducts or soft tissue or something is growing inside the ducts like early cancer especially at your age of 49. Check with your doctor if it does not improve after a week with daily warm compress and topical antibiotic.
