Difficult to say: It is unlikely to find a common symptom that presents first in paranoid schizophrenia. Problems socially or different perceptions of the world may exist long before true paranoia, hallucination, or delusion. Since the disorder is usually diagnosed in the late teens for men and early 20's for women, odd or unusual, even bizarre thinking may be present before the full disorder is identified.
