Very difficult: I recommend taking him to a specialized center or tertiary care center. There are many reasons why this condition happens and the treatments will be different. The initial treatment is to thin the blood.

Portal hypertension: I would have him be evaluated by a tritiary care pediatric hospital with experience in liver transplantation and liver disease. Perhaps he can get a surgical shunt placed or a tips if he needs it. Definitely needs expertise evaluation.
