Toe pain?: Although its a scary situation, it's not an "emergency" where he's at risk of dying within a few hours or days. Recommend calling for an appointment within your insurance network at first available or if pain and drainage is noticeable, go to an urgent care for initial treatment, then keep appointment with a podiatrist.

Fever or warmth? ER: If the injury was a week ago, and now the toe is red and swollen, there should be some concern for either a skin infection or an underlying fracture. The great toe is the most important one, as it is essential for walking. If he has fever, or the redness is 'streaking' up his foot, he should be seen sooner. Call his pediatrician, or go to an urgent care or er. Ice, elevate and Ibuprofen until seen.
