Knee strain: First rice, rest ice compress(wrap) and elevate. Time and the correct support to stop re-injury are the main treatment in additional to anti-inflammatory meds. Prolotherapy may help.

Depends: It depends on the severity of the injury. Mild to moderate injury is almost always physical therapy. Severe injury or complete disruption is usually addressed with operative repair.
