I have small fiber neuropathy 2nd skin biopsy shows igm deposits in vessels. also says microvascular injury syndrome. is this autoimmune process?

Complicated: There are both immune-mediated and neoplastic conditions, such as waldenstrom's macroglobulinemia and others, that can cause similar pictures. Please have an evaluation by a hematologist. Some blood tests to look for monoclonal proteins, radiologic studies, and possibly a bone marrow biopsy may be necessary to figure out what is going on. The picture needs to be put together.

May be very complex: Small fibre neuropathy can be encountered with celiac disease, diabetes, pernicious anemia, fibromyalgia, alcohol/nutritional issues, occult neoplastic processes (as mentioned), a variety of "paraproteinemias", lupus and sjogren's disease, some of which will have immune deposits involved. Most of these situations are immune mediated and may respond to ivig. ?Visit local medical school for advice?
