Bleeding tongue: A bleeding lesion at base of tongue persisting for that long with your smoking history must be biopsied and imaged. Go see ENT to check for possible cancer.

Urgent care: You need to see an ear nose throat specialist as soon as possible. Anything that goes on for as long as yours has could be something serious and needs to be addressed as soon as possible.
