Knee swelling: Knee swelling has nothing to do with the location of the pain. The knee, like most joints, has a capsule that surrounds the joint and keep the fluid contained. When there is an excess of fluid due to injury or inflammation, then the knee will swell. You will usually notice the swelling either above (proximal) the knee or the lateral aspect as these are weaker/thinner areas of the capsule.

Swelling: Swelling ina knee joint may show up first laterally since there is not a muscle on that side close to the knee cap and it is more visible. The entire knee joint fills with fluid when it swells and the fluid can move around.
