Analyze your swing: Have someone who knows what they are doing analyze your swing with the various clubs. Usually you can figure out where in your swing you are putting stress on your elbow and then you can figure a way to adjust your swing.

A lot: Golfers elbow aka medial epicondylitis is a chronic tendonitis of the inner portion of your elbow. The first things to do are rest, ice, and anti inflammatory meds. A program of home exercises are helpful - see http://nycsportsmed.Com/pdf/nitric%20oxide%20for%20lateral%20epicondylitis.Seneviratne-2.Pdf if you don't improve - see your sports medicine doc for further evaluation and treatment.
