Many possibilities : Blood in the urine can be caused by a cancerous tumor, a bladder polyp, an infection, interstitial cystitis, a kidney stone, and kidney disease. Additionally, blood can also be present on a urinanalysis from extra-urinary sources such as vaginal orrectal bleeding.
