See a doctor: First and foremost i'd rule out a cellulitis or dvt. If all else checks out, and you have pain from back issues etc then follow up with pain management. May consider spinal cord stimulator instead of all those pain killers your taking.

Leg modification: Complex regional pain syndrome. It is difficult to explain and to understand. Usually follows a major injury, operation, foreign materials to fix a fracture, etc. Has a sympathetic component (this is why the increased temperature and redness) unilateral sympathetic nerve block might help along with hot.Cold, tens, physical therapy, spinal stimulator. Prevention of bone loss is indicated.
