Can there be a problem with being narcissistic or why is it called personality disorder?

Yes, BIG problem: A personality disorder is an enduring, pervasive, and inflexible pattern of inner experience and behavior that differs markedly from one's culture, begins in adolescence or early adulthood, and causes distress in important areas of life (relationships, work, etc). Narcissistic pd causes problems b/c they are grandiose about themselves, need constant admiration, and can't empathize with others.

Narcissism: Personality disorders in general are at higher risk for suicide. Narcissists eventually drive everyone around themselves away through the toxicity of their very presence. and cannot see or understand their part in their own misery, which leads eventually to depression and possibly suicide. The core of it is they cannot face their own abysmal self-esteem or the reasons for it.
