Possible dry socket: Have your oral surgeon take a look if the pain is that intense and otc pain meds are not helping you.

Dry Socket... or...?: When the area of a tooth extraction all of a sudden a few days after the extraction starts to hurt more, it typically results from the loss of the blood clot within the socket, exposing bone and causing increased pain and is known as a dry socket. Other less likely possibilities are food trapped in the socket or an infection. See your dentist asap as this is usually quickly and easily remedied.
