See a dermatologist: Difficult to tell without an exam. Bald spots on scalp can be caused by alopecia areata (condition where immune system attacks hair follicles), fungal infection of scalp, even child pulling out hair, many causes. White patches could be vitiligo (also autoimmune like alopecia areata-two conditions sometimes run together), a form of eczema, called pityriasis alba or a rash like seborrheic dermatitis.
