How can we distinguish orthostatic hypotension from vertigo? seated for one hour, then stood up, then feeling dizzy or loss of balance?

A traditional physical exam can easily point the way. Blood pressure taken after lying down, sitting up, and standing will establish an orthostatic drop. Balance, and coordination testing, exam of eye movements supine, laying on both right and left sides, and standing. Much more can be done but this is a start.
