The woman's dentist will determine which antibiotic alternative is best and then prescribe it. Extractions with local anesthetic should pose no issues. Since the tooth is hopeless and infected, the sooner it is out the better off her overall health will be.

The antibiotic of choice when allergic to penicillin is 500mg of azithromycin. Most doctors will wait for the second trimester to do an extraction in order to avoid complications, but if the tooth is loose or a bad infection is present than an extraction is indicated. Good oral hygiene is necessary during the full term of pregnancy.
