The bladder size & location is common to all & easily located/sized using ultrasound. The retention balloon that is part of the catheter is small and usually just 2-3X the diameter of the catheter, preventing its accidental loss through the insertion portal.

The catheter has two holes at one end and is inserted in an already formed tract till urine comes out and then the balloon is inflated. There is always a small round cavity there to accommodate both and a small amount of urine is there all the time to the level of the holes at the tip. I do not undersatand the last part of your question. PL rephrase
