Having underlying heart disease is what makes PVCs most concerning. I don't think interpolated PVCs are any more dangerous than other PVCs( which have a compensatory pause). Very frequent PVCs, R on T PVCs, couplets or runs of PVCs, multifocal PVCs are concerning to me and warrant an evaluation by a cardiology electrophysiologist even in the absence of underlying cardiac pathology.
