Assuming the comment referred to the lateral leads on the EKG (V5 and V6), there are numerous possible causes, including left ventricular hypertrophy or dilatation, coronary artery disease, electrolyte abnormalities and others...that's why it's termed, "nonspecific". To further clarify its significance, further exam, history and tests would be required.
