Biologically, most antibiotics, including metronidazole and doxycycline, start to work within hours. Symptoms improve much more slowly; you should not expect improvement until at least 3-5 days after starting the drugs. For PID, pelvic pain and vaginal discharge usually are gone after 8-10 days, sometimes longer. No improvement by 2 weeks, usually means PID wasn't the cause of the symptoms.
