The ultrasound report identifies a mass consistent with either a fibroid tumor or a polyp (benign "neoplasms") which may be the reason the ultrasound was ordered, as both may cause bleeding between menstrual events. See your GYN physician who ordered the ultrasound for guidance. "Free fluid" in the abdominal cavity is a frequent and essentially normal finding.
