The words "chronic" & "small vessel ischemia" imply poor blood flow leading to poor nutrition of the tissue leading to tissue changes. It does not suggest a stroke.

These are typically descriptions of non-diagnostic and non-specific T2 scattered lesions, which may be present due to micro-vascular angiopathy, smoking, aging, migraine, recreational drug use, various medications, prior head injury, etc. Your post does not describe a specific finding of a stroke, but ask the radiologist if you are concerned.
