Serum sodium should be high (hypernatremia) when blood is concentrated and urine sodium should be below 20 mmol/L when volume is low (hypovolemia). With the given info, I'm afraid that you may have 2 separate conditions, # 1 erythrocytosis (high altitude, chronic lung disease, high erythropoietin, etc.) and #2 problem with diluting the urine such as SIADH.
