The interpreter is commenting on the prominent size of Q waves in limb leads 3 & AVF which can be findings in a scar in the inferior wall of the left ventricle. However, to diagnose a probable old inferior wall infarction ( scar) one would have to also see a prominent and wide Q wave in limb lead 2, the other limb lead ( in addition to 3&AVF) reflecting the inferior wall of the left ventricle
