I have a small hole next to nipple that disappears and blends in with nipple when hard. what can this be? doctor did a breast exam recently and found nothing abnormal, no lumps? no redness, areola looks otherwise normal?

It's probably a gland opening, either in the skin or the breast itself (which is fundamentally just a giant gland). When the nipple contracts and gets hard the opening is squeezed closed.
