It very much depends on the etiology, e.g. if low EF is due to ischemic cardiomyopathy and coronary artery bypass surgery is feasible then surgery is indicated. Cardiologist and CT surgeon should give you the appropriate advice for making decision. Good luck.

In general meds used in congestive heart failure with reduced ejection fraction are intended to reduce the volume load on the heart through different mechanisms. Unless there are significant valvular abnormalities, surgery is not an option, pacemakers and revascularization, when possible are still used in specific cases.
