You would know it is getting worse from a combination of worsening symptoms and worsening imaging studies. This is why it is important to follow up with your neurologist so that the neurologist, who knows you best, can evaluate your situation regularly. That doctor is likely to pick up on subtle changes that could indicate a worsening condition. Good luck to you!

If you truly still possess a Chiari Type 1, the described symptoms are not due to the congenital malformation. Would suggest treating the migraines at the office of a Medical Neurologist, and also, complying with followups with your neurosurgeon.
