Olmesartan is less likely the cause of isolated increase in ALT but it can't be rule out completely. Isolated increased in ALT is quite common which may be due to alcohol intake or taking acetaminophen on the night before blood test or it may happen without an apparent cause. You should have the test repeated and do not take alcohol, acetaminophen, supplements, etc. on the date before the test.

Although practically any medication can affect the liver in someway, it is not likely that this medication is the cause of your elevated ALT. Liver disease can cause a rash which resembles hives, but the real answer is to find what is the cause of your liver problem. there are many potential causes of elevated ALT, which needs to be sorted out by a liver specialist. I fully agree with the advice given, but in view of the hives&increased ALT, it may be worth stopping olmesartan & using a different class of medicine such as a calcium channel blocker like amlodipine and seeing if hives resolve and ALT normalizes.
