Probably you have cirrhosis, i.e. serious scarring of liver tissue. However, ultrasound diagnosis usually requires confirmation by other tests, like liver biopsy. There are several possible causes of cirrhosis; discuss with your doctor. (Are you are misreading "observation"? Maybe "obstruction"?) Good luck.
