It means your thyroid is not producing enough thyroid hormone to tell pituitary not to kick out TSH. So you need to be taking thyroid probably or ic you are taking u need more. Discuss with Dr who ordered tests.

If your TSH is high and your T4 is low, your pituitary senses that the thyroid hormone levels in your blood are low. People with Hashimoto's thyroiditis generally take a T4 pill every day, but the dose often needs to be adjusted.
