Had a yearly physical exam at my dermatologists. had to get something frozen and an injection for alopecia. i get a large bill of a couple hundred bucks because i havent met the deductible. these kinds of things arent usually covered in a physical?

No, your visit included procedures by a specialist that are billed aside. Physicals are only billed by primary care docs annually.

Every insurance plan is different;depends on your individual plan as to what is covered.Most plans only cover yearly physicals for well care (with your primary care physician, and/or a gynecologist),not specialty care with a dermatologist.For those visits, even if it is your yearly visit for a skin check,there is generally at least a copay for the visit (in network) and extra for any procedures.
