Sometimes when running before or after when stood up i get a sharp cramping pain around my heart that lasts a couple of seconds then it goes away it happens randomly not every day what could this be im 23 and physically fit?

Could be reflux. Even GI cramping but even if young would get and EKG and cardiac evaluation ( consider echo) because there are some congenital heart issues that may not present till later in life like hypertrophic myopathy., mitral valve prolapse.
