Find the cause first: Start with the cause for overloading the anterior knee (tight hamstrings, flat feet, weak quads) and address these first to prevent a recurrence. Initial treatment focuses on rest, unloading with a patellar band, pt, orthotics, and oral anti- inflammatories (if tolerated). I am not a big fan of steroid injections near the tendon due to risk of rupture. Prp is promising but still in its infancy.

PRP: It is very effective and works wonders. It stimulates healing while cortisone does the opposite.

Direct comparisions: are difficult. But, remember PRP is regenerative while cortisone is degenerative. Which one sounds like it would be better for you?
