Last month i had free fluid in the adnexa region aspirated. this month during the follow-up visit the fluid is back and even larger than before. there is no cyst. what could be causing this? note: i had endometriosis surgery almost two years ago?

A spontaneous rupture of an ovarian cyst (or two) can cause free fluid in the area behind (posterior to) the uterus. Ask your GYN physician for guidance.
