The most likely cause is infection or inflammation in the scalp around the occipital lymph nodes drainage. Since it is painful to touch, warm salt water compress application for 5 -10minutes twice daily might help. It is best , however, to see the PCP for evaluation and appropriate management.

Cancer is unlikely. You may feel the nodes every couple of weeks, if any of the lymph glands doubles in size, becomes painful, ulcerates, or other nodes become enlarged, or you develop fever, or lose weight, you should see a doctor. Wish you good health!
