Migraines with aura are associated with an increased risk of stroke, particularly in women who are using certain forms of hormonal contraception. Calcitonin gene-related peptide (CGRP) is a protein that is involved in the regulation of pain and inflammation in the body. There is currently no evidence to suggest that CGRP inhibitors such as galcanezumab (Emgality) increase the risk of stroke.

Triptans, which can abort migraines, have been documented to cause both heart attacks and strokes, but there is no evidence whatsoever to implicate the CGRP class as a cause of stroke. Your classical migraine stroke risk would prohibit your blithely using birth control pills and smoking.
