Bactrim should be avoided in patients with chronic kidney disease. If there's no alternative, it can be used with adjusted dose based on eGFR and renal function should be checked during or after treatment. The treatment course should not be more than one week. Bactrim DS, twice a day is a full dose, not adjusted yet, you should recheck with the prescribed doctor for alternative or dose adjustment.
