54 year old obese male; had a heart attack 3 years ago; has had diabetes and chf stage c for 7+ years; bad edema, shortness of breath, immobile; takes beta blockers, diuretics, etc. but won't change diet, exercise, or see a cardiologist. prognosis?

Not a good prognosis. I am afraid that this male patient, if he continues on this path, without lifestyle changes and without the care of a cardiologist will almost certainly have another heart attack which may turn out to be fatal.
