A 3 yo girl with this pattern may have retained foreign material in their vaginal opening, or irritation about the pee hole. This often occurs with tub baths where there is lots of soap/bubbles. A gentile exam of that area would be prudent. It could be something else & an evaluation by her doc can be helpful.
