A sixty year old male presented with great toe ulcer. there are marked soft tissue swellings of hallux, with diffuse irregular loss of medullary and cortical bone of the distal phalanx. can someone help in identifying the pathology present here?

This patient has osteomyelitis of the big toe, quite commonly seen in diabetic patients. Surgery is indicated for this type of infection but vascular work-up should be done first for deciding the best surgical option if patient has diabetes mellitus or any sign of peripheral vascular disease.

It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. If the person is diabetic, a diabetic ulcer would be a top consideration, the other high likelihood pathology is gout. It would be prudent to see a doctor. Wish you good health!
