Makes no sense: I agree, something else is going on. Sounds like there has been boney damage and possibly osteomyelitis. Not scary because whatever it is is limited but a new x-ray is indicated.

Delayed is not fine.: You did not describe the nature of the toe fracture or whether or not you had surgery to correct the fractured bone. Bad great toe fractures can take more time to heal but 16 weeks is pushing the limit. I think a second opinion might set your mind at ease as well as verify a potential situation before things get worse. To remove all doubt, have it checked out.

MRI: I would start with a new x-ray. If still negative, an MRI might find something. "Extreme" swelling at this point is unusual. Dr. Latva
