Ive been on 75mg fentanyl patch for over a year with 1-2 10mg hydrocodone for breakthrough pain. im fighting metastatic stage 3c ovarian cancer and also recently diagnosed with ra. it doesnt seem to work as well. am i developing a tolerance?

Developing opiate tolerance is always possible having other sources for pain that are not responding as well to opiates, inflammatory pain as seen in RA being an example is as well possible. For such pain specific RA directed meds may be needed to control RA along with short courses of steroids and before considering escalating or rotating opiates.

Your fentanyl patch dose is 75 mcg/hour & not mgs. There's only one stronger strength at 100 mcg/ hour but your oncologist will have to advise you about dosage. I'm unsure as to how much your RA is contributing to your discomfort. If it's considerable a rheumatologist will have to be involved significantly in your care. It's important that you achieve enough pain relief. ?See pain management MD
