TCA for IBS: Amitriptyline, a tricyclic anti-depressant, is in treatment of multiple conditions (eg ibs, insomnia, migraine) & not just depression. Nortriptyline is an active metabolite of amitriptyline. They may both work equally in treatment of ibs as well insomnia in the same individual. In my opinion, Amitriptyline dosing is more predictable, is more sedating & can cause more weight gain than nortripyli.

Both are used: But amitrypiline gets my vote based on my personal experience over almost40 years and the other amount of use of this med for other uses than only depression. Nortyptilline is an ok antidepressant for the right patient.
