Hiatal hernia does not present with pain in the lower abdomen. Abdominal pain is the main symptom of Crohn but changes in bowel movement such as loose, frequent stool with blood and mucous are also present. I wonder what medication is effective in relieving the pain, is it an antispasmodic? Irritable bowel syndrome (IBS) is more consistent than inflammatory bowel disease (IBD) in your case.
