The process of smoking 'fully' usually involves drawing the smoke deeply enough that it is distributed inside the air exchange centers of the lung. This exposes the alveoli (air exchangers) to the dozens of tiny particles of ash and the chemicals in the smoke. Just mouthing the smoke has fewer negative consequences and most do not consider it smoking.
