Feel a throbbing in right back of head, i feel a rocking dizziness when it throbs. upper shoulder pain/strain. no neck pain or headaches. brain and neck mri was normal. somehow ears are red, inflamed and painful according to pcp. no allergies?

You state you have no headaches but still complain of a throbbing discomfort in the right side of your head. I would suspect a migraine like syndrome here. The vasodilation or redness in the ears can be the red ear syndrome seen in migraine. If you haven't consulted a neurologist or headache specialist, I urge you to do so.
