Varicose veins can cause discomfort in the scrotum and seldom cause the symptoms you mention. If you continue to hurt pl see a Urologist. In the meantime wear a good scrotal support and use local heat. It does not affect testosterone and affects sperm counts only if of long duration- and then not in everyone. hope you feel better
