She's too young to have coronary artery blockage by plaque but could be experiencing coronary artery spasm provoked by exertion. I recommend an evaluation by a cardiologist which would include a treadmill stress test and wearing of an outpatient cardiac monitor to check her heart rhythm during palpitations and look for ischemia during exertion. A lipid panel & thyroid levels are worthwhile.
