Genitourinary pain can be caused by infection, trauma or other possible causes of inflammation. It merits an urgent visit to one's doctor or to a urologist. A careful history and physical exam, with a urinalysis and blood tests including a CBC, kidney tests and PSA (prostate specific antigen) will usually reveal the likely cause and appropriate treatment.

Sounds like prostatitis. He should be examined by a Urologist. It may temporarily affect fertility if it is an inflammation
